Retinal Detachment vs Retinoschisis
RD
• Corrugated surface (unless chronic)
• Presence of demarcation line
• No laser uptake
• Unilateral
• High myopia
• Relative visual field defect/ scotoma
Retinoschisis
• Smooth domed shaped surface, no demarcation line, good laser uptake, bilateral, hyperopia, absolute visual field defect (can be tested by focusing slit beam onto affected area)
• The laser energy is taken up by the RPE layer, but the reaction is in the inner retina
• Therefore, the laser uptake is good in retinoschisis and absent in a retinal detachment
• Retinoschisis affects 5% of population
• Patients are asymptomatic unless posterior extension causes significant field defect
• They most commonly occur inferotemporally, arising from areas of peripheral cystoid degeneration
• Combination of inner leaf and outer leaf breaks can lead to schisis detachments
