Hydrops: Managed Medically

A case of acute hydrops managed medically

•A 49 year old male, known case of keratoconus came with complaints of blurring of vision and redness and irritation since 1 day.

•He gave a history of eye rubbing.

•On examination, the superior cornea showed edema, confirmed on AS OCT as acute corneal hydrops with a break in the Descemet's membrane, leading to fluid clefts in the stroma.

•Patient was given the choice of compression sutures or conservative management.

•He was not keen on surgical management and opted for medication.

•And was started on topical hyperosmotics and low dose topical steroids in a tapering dose.

•At 6 months, the hydrops had scarred with a notable improvement in vision — a reminder that in selected cases, conservative management can yield excellent results
