Worth 4 dot Test

The Worth Four Light Test (Worth's Four Dot test, W4LT) is a clinical assessment used to evaluate binocular vision, including detection of diplopia, suppression, and anomalous retinal correspondence (ARC). It is simple, quick to perform, and can be used in verbal children.

• The apparatus for this test consists of a box containing four panes of glass, arranged in a diamond formation, which are illuminated internally. The two horizontal panes are green, the upper one is red and lower one is white.

• The patient wears red and green goggles.

• The Red goggle is placed in front of right eye.

• The Green goggle is placed in front of left eye.

• The test can be performed separately for distance and near vision.

INTERPRETATION OF RESPONSES

1) The patient sees all the four dots:

-Normal retinal correspondence with no heterotropia.

-Harmonious anomalous retinal correspondence (ARC) with manifest squint.

2) 2 red dots: Suppression of left eye

3) 3 green dots: Suppression of right eye

4) 5 dots, red dots appear to the right: Uncrossed diplopia with esotropia

5) 5 dots, red dots appear to the left of the green dots: Crossed diplopia with exotropia
