AC wash for Hyphema

A 60 year old male came with history of trauma to right eye

On examination he had hyphema in the AC with IOP of 36mmHg

He was given mannitol 100cc

His IOP and hyphema did not reduce over the next few days and increased to >35mmHg inspite of being on AGM

So, he was taken up for an AC wash.

Procedure:

-Side port entry was made at 3 o clock

-The blod clot was removed with simcoe's irrigation and aspiration cannula

-Antibiotic wash given

-Side port sutured

Post operatively, IOP reduced significantly and patient is now comfortable

Hyphema AC Wash Indications:

• Corneal blood staining

• Significant visual deteoriation

• To prevent optic atrophy (IOP > 60mmHg for >48 hours despite max medical therapy)

• To prevent PAS (Hyphema <50% for 8 days)

• IOP >25mmHg with total hyphema >5 days

• IOP >24mmHg for >24 hours (or transient IOP >30mmHg) in patients with sickle cell disease/ trait
