Arcus Senilis
Bilateral grey to white band of peripheral corneal opacification.
FEATURES:
Bilateral.
Fine dots with a clear zone between it and limbus (clear interval of Vogt).
Diffuse central border with sharp peripheral border.
Begins superiorly & inferiorly and involves entire periphery.
Asymmetric in Unilateral Carotid vascular disease ( decreased arcus ) & chronic ocular hypotony (increased arcus)
LOCATION:
Begins in DEEP STROMA with progression to superficial stroma.
COMPOSITION:
Extracellular corticosteroid esters of lipoproteins of low density.
Lipid material leaks from limbal capillaries with central flow being limited because of a functional barrier to the flow of large molecules on the cornea.
ARCUS JUVENILIS:
Associated with increased plasma cholesterol and LDL levels in men younger than 50 years.
Increased risk of mortality from CHD and Cardiovascular disease
Increased risk for type 2a dyslipoproteinemia.
